Notice of Privacy Practices

Effective date: Pending final approval

This notice describes how medical information about you may be used and disclosed, how you can get access to it, and our responsibilities. Please review it carefully.

This notice applies to health information created or maintained by Blazing Stallion Digital, LLC in connection with chiropractic services provided under the public-facing name The STRONG Clinic.

Your rights

Get a copy of your record

You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you. We will ordinarily provide a copy or summary within 30 days and may charge a reasonable, cost-based fee when permitted by law.

Ask us to correct your record

You may ask us to correct information you believe is incorrect or incomplete. We may deny a request when permitted by law, but we will explain the reason in writing, ordinarily within 60 days.

Request confidential communications

You may ask us to contact you in a specific way or at a different address. We will accommodate reasonable requests.

Ask us to limit use or disclosure

You may ask us not to use or share certain information for treatment, payment, or healthcare operations. We are not generally required to agree. The STRONG Clinic is cash-pay and does not presently submit health-insurance claims.

Get a list of disclosures

You may ask for an accounting of certain disclosures made during the six years before your request. The accounting does not include every disclosure. One accounting in a 12-month period is provided without charge.

Get a copy of this notice

You may request a paper copy at any time, even if you agreed to receive it electronically.

Choose someone to act for you

A person with legal authority to act for you, including a parent or legal guardian, may exercise your rights. We may verify that authority.

File a complaint

You may complain to us or to the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Your choices

Unless you instruct us otherwise, we may use professional judgment to share information relevant to your care or payment with a person involved in your care. We do not sell patient health information. Marketing messages are optional and are not a condition of treatment.

How we typically use and disclose information

When legal conditions are met, we may also use or disclose information for public health and safety, health oversight, research, workers’ compensation, law enforcement, court proceedings, required reporting, and other purposes permitted or required by law. Other uses require written authorization, which may generally be revoked in writing.

Our responsibilities

Mobile-practice privacy

We use reasonable safeguards during scheduling, arrival, treatment, transport, documentation, and communication. Tell us about reasonable privacy preferences, including calling before approaching or avoiding mention of chiropractic care at the door.

Calls, text messages, and ordinary email may be used for scheduling, reminders, receipts, and basic administrative messages according to your preferences. Voicemail is used only when you permit it, and messages contain as little health information as reasonably practical. Detailed symptoms, diagnoses, records, and treatment discussions are handled in person, by telephone after appropriate identity verification, or through an approved secure system. Marketing messages require a separate optional choice and are never a condition of care. You may change your communication preferences by contacting the clinic.
